Preparing for your visit:
We are looking forward to seeing you in the clinic. To make your visit most productive, here's some information about how to be prepared:
Before scheduling your appointment, we strongly recommend you contact your insurance company to verify that the Empowered Arthritis and Rheumatology Center or the doctor you plan to visit is included in your network.
Your insurance company will also be able to inform you of any co-payments, co–insurances, or deductibles that will be your responsibility. If you proceed in scheduling an appointment and your health insurance benefits do not participate with EARC, your out-of-pocket liability may be higher. We will contact you regarding your coverage and patient liability.
At this time, we are in network with the following insurances:
- Blue Cross Blue Shield
- Tricare-Humana Military East
- Three Rivers Provider Network (TRPN)
- First Health
- Coventry-WellPath healthcare
We are not in network with Medicaid and cannot accept any upfront payment from such patients as per CMS rules. Please call our office to confirm prior to the appointment as there are some other insurances where we are not in network.

Frequently Asked Questions
Depending on your insurance, you may need a referral from your primary care physician, but many patients can make appointments with us without referrals.
